What to bring to your first visit

Arriving a few minutes early with these items helps us get you seen on time.

  • Photo ID (driver's license or state ID)
  • Insurance card(s) — including Medicare/Medicaid if applicable
  • Completed new-patient forms (or arrive 15 minutes early to complete them)
  • A list of current medications & dosages
  • Immunization records (for children & new patients)
  • Any recent lab results or records from a previous provider
