Medicare

We proudly care for Medicare patients, including preventive wellness visits and chronic care management.

Medicaid / AHCCCS

Arizona Medicaid (AHCCCS) members are welcome, and eligible children can receive VFC immunizations.

Workers' Compensation

We provide evaluations, treatment, and documentation for approved work-related injury claims.

A few things that help every visit go smoothly.

  • Bring a current insurance card & photo ID to every visit.
  • Copays are collected at the time of service.
  • Some services may require a referral or prior authorization from your plan.

Do you take my insurance?

We're contracted with most major insurers plus Medicare, Medicaid (AHCCCS), and workers' comp. Networks vary by plan, so call us at 602-569-5437 with your card and we'll confirm your coverage before your visit.

What if I don't have insurance?

We welcome self-pay patients and are glad to discuss options. Eligible children may also qualify for low- or no-cost immunizations through the Vaccines for Children (VFC) program.

Will my child's vaccines be covered?

Many childhood immunizations are covered by insurance as preventive care. For eligible children who are uninsured, underinsured, Medicaid-enrolled, or American Indian/Alaska Native, the VFC program helps cover recommended vaccines. We'll help confirm eligibility.

How does workers' comp billing work?

For work-related injuries, please bring your claim information and employer/insurer details. We coordinate billing directly with the workers' compensation carrier for approved claims.
